MY LABS FOR LIFE, LLC reserves the right to modify or make changes to the terms and conditions at any time. Changes will be effective immediately upon posting of the modified terms and conditions. Permission to use this site is at the discretion of MY LABS FOR LIFE, LLC and may be terminated at any time for any reason. You agree that you are over the age of 18. You agree that the information you submit during the course of purchasing a lab testing from MY LABS FOR LIFE, LLC is truthful and accurate. I have read and agree to the Terms and Conditions and the Privacy and Security Policy. I understand that unused requisitions for orders will no longer be valid after thirty days from the original purchase date, no credit or additional services will be provided in exchange for unused requisitions after thirty days from the original purchase date. I also acknowledge and agree that for privacy protection MY LABS FOR LIFE, LLC will have the lab results emailed to the email address placed on the purchase order by the consumer being tested. Should the consumer being tested elect to share those results, it will be up to him or her to do so directly.
