Direct-to-consumer lab testing from any resource cannot be submitted for insurance reimbursement.
We cannot give out CPT codes for any purpose.
Can I use my HSA card? Possibly

Do I need to visit the doctor to get my own labs? No
A visit to the doctor is not required to order your own labs.
Can anyone be tested? No
You must be 18 years or older to obtain lab tests with MyLabsForLife.

Is testing available in all 50 States? No

Return Policies
The return policy for a refund is 90 days from the date of purchase.
The kit must be returned to the lab before any refunds can be issued.
Shipping fees of $19.95are non-refundable.
There is a $25 restocking fee.
Credit card refunds can take up to 30 days to be processed.

Any information collected and/or retrieved by MyLabsForLife.com is encrypted via an SSL connection to protect and safeguard your information from third parties. The only information we may keep is your email address if you opt-in to periodic e-newsletters. We do not under any circumstances give your email address to third parties.
Specialty Tests Kits. Due to state laws, test kits cannot be sent to NY, NJ or RI. Please contact us to discuss this issue. Test kits will be sent to the address listed on your order form. It normally takes 5-7 days to receive a test kit. Follow the sample collection directions that come with the test kit. Everything you need to send the test kit back to the lab is contained within the test kit. The results will be emailed to you when the test has completed processing.
